Signature pages 

• Pre-submission Supplementary Signature Page (PDF)
  Provides additional space for the University's required signatures—Principal Investigator, Co-investigator(s), Principal Investigator’s Department and Faculty, and Research Services Office (RSO)/Research Facilitation Office (RFO)/Institution the application does not accommodate them. In addition, the Pre-Submission Supplementary Signature Page MUST be completed when there are University of Alberta co-applicants and an institutional signature is required.

  Requests a new project to be used for conference funding. Identifies various sources of support, the date, location and purpose of all conferences, workshops and seminars. 
• Capital Health Authority Salary Project (PDF)

  Requests a salary project funded strictly by Capital Health Authority.

Request for changes to existing projects

• Change to Existing Research Project (PDF)
  Requests an extension, renewal or amendment of the terms of an existing research project, including funds to be deposited to "General Research."

  Requests a change of project holder of an existing research project.
