Final Evaluation Forms for Use by Mentor Teachers
Below are links to the Evaluation Forms used in the Field Experiences. The forms are password protected MS Word documents. When you click on a link to a form you will be required to type in the password provided to the Mentor Teacher by the University Facilitator. The password is case sensitive. Either click on the OK button or press the Enter key to open the document.
- You may work on the form or save it to your desktop or documents folder to work on it later.
- To save the document select either Save or Save As from the File Menu.
- You may rename the document and select the location where you want it saved.
- If you do not want to have to use the password to open your document click on the Options button in the Save Window.
- Delete the ***** appearing in the password to open box
- Click OK before saving your document.
